first_imgPresident Trump presented a broad-brush outline of how his administration hopes to stem the decades-long increase in prescription drug prices and spending, in a speech Friday in the Rose Garden of the White House.The administration also released a 39-page document describing a variety of proposals it is either considering or studying in an effort to lower costs to individuals, corporations, the government and the economy as a whole.There were few concrete actions taken today. The plan mostly lists dozens of “potential” steps the administration may take, along with many others that were included in the president’s budget proposal and will require congressional action. Several options are “requests for information.”Nevertheless, the president characterized the proposal as “the most sweeping action in history to lower the price of prescription drugs for the American people.””We will have tougher negotiations, more competition and much lower prices at the pharmacy counter and it will start to take effect very soon,” Trump said.The president was accompanied by Health and Human Services Secretary Alex Azar, who played a major role in developing the plan.Trump said he wants to eliminate “middlemen” in the drug industry — which includes pharmacy benefit managers and wholesalers — and he criticized industry lobbyists for making a fortune at the expense of taxpayers and patients.U.S. patients spent nearly $329 billion on prescription drugs in 2016, according to the National Health Expenditures Survey. That’s an increase of about $100 billion in the past 10 years.Many researchers say the biggest reason behind the rising spending is high prices for prescription drugs. Often, companies are rewarded for setting a high retail price because insurers negotiate discounts off that initial price.The blueprint released Friday reiterates proposals that were included in the White House budget proposal. Those include allowing Medicare’s prescription drug plans to slim down the number of drugs they pay for to spark price competition among pharmaceutical companies, and capping how much money Medicare patients themselves can spend on drugs each year.”On the positive side, I will say that HHS does seem to be paying close attention to this issue, given all the issues they bring up in the blueprint document,” says Walid Gellad, director of the Center for Pharmaceutical Policy and Prescribing at the University of Pittsburgh.”On the negative side, it’s a bunch of questions, not a specific plan for how to proceed.”Azar, in a briefing following the Rose Garden speech, acknowledged the plan is sweeping and would take years to implement. He said the administration will pursue those ideas it can get done.”This is a major restructuring of a huge portion of the U.S. economy. One doesn’t do that lightly,” he said.The plan, like the budget, also proposes limiting price increases Medicare would pay to the rate of inflation and changing some rules around generic drug approvals to prevent companies from blocking new generic competition.Many of those ideas would require legislation and congressional approval.Azar, in his briefing, emphasized his desire to change the way pharmacy benefit managers operate — specifically a system where they negotiate rebates off high drug prices.”We are calling into question today the entire structure of using rebates in the pharmacy channel,” he said.The president also wants to take steps to ensure foreign governments don’t get lower prices for drugs than do U.S. taxpayers – which he referred to as “global freeloading.””It’s unfair, it’s ridiculous, and it’s not going to happen any longer,” he said.Critics say that proposal is unlikely to help lower prices here in the U.S.”Lifesaving medicines aren’t more expensive here because they cost less elsewhere,” Jason Cone, executive director of Doctors Without Borders USA, wrote in an article published in The Hill. “They’re priced out of reach everywhere because pharmaceutical corporations are charging exorbitant prices simply because they can—and the U.S. government lets them.”One thing Trump did not propose was allowing Medicare, the government health care program for the elderly, to directly negotiate lower drug prices for its beneficiaries. That’s a change from his rhetoric during his campaign and transition.”We’re the largest buyer of drugs in the world and yet we don’t bid properly,” he said at a news conference in early January 2017. “We’re going to start bidding and we’re going to save billions of dollars over a period of time.”But shortly after Trump moved into the Oval Office he wavered on that commitment, first saying that he didn’t want Medicare to engage in what he called “price fixing” and then, through his former spokesman Sean Spicer, saying once again he favored allowing the federal government program to deal with drugmakers to get lower prices.Medicare accounts for about one-third of U.S. prescription drug spending. Current U.S. law prohibits Medicare officials from interfering in the negotiations between drugmakers and the insurance companies that administer Medicare’s prescription drug plans.The only government report that looks at the issue is a 2007 Congressional Budget Office study that concluded it would have a “negligible effect” on prices.But Gellad says that report was limited, because it looked only at the effect if Medicare were not allowed to exclude drugs from its coverage list.”There is no evidence that allowing Medicare to negotiate doesn’t help,” says Gellad. “In fact, the CBO said that in certain circumstances, with the right formulary, this type of negotiation could help.” Copyright 2018 NPR. first_imgThere’s new evidence that a woman’s levels of female sex hormones, including estrogen and progesterone, can influence her risk of Alzheimer’s and other forms of dementia.Women are less likely to develop dementia later in life if they begin to menstruate earlier, go through menopause later, and have more than one child, researchers reported Monday at the Alzheimer’s Association International Conference in Chicago.And recent studies offer hints that hormone replacement therapy, which fell out of favor more than a decade ago, might offer a way to protect a woman’s brain if it is given at the right time, the researchers said.The findings could help explain why women make up nearly two-thirds of people in the U.S. with Alzheimer’s, says Maria Carrillo, the association’s chief scientific officer.”It isn’t just that women are living longer,” Carrillo says. “There is some biological underpinning. And because of the large numbers of women that are affected, it is important to find out [what it is].”Scientists have long suspected that sex hormones such as estrogen and progesterone play a role in Alzheimer’s. And two studies on dementia and what occurs during a women’s reproductive years support that idea.One of the studies looked at nearly 15,000 women in California. And it found an association between a woman’s reproductive history and her risk of memory problems later in life.The risk of dementia for women who had three or more children was 12 percent lower than the risk for women who had one child, according to Paola Gilsanz of Kaiser Permanente Northern California Division of Research, and Rachel Whitmer of the University of California, Davis.Also, women who began to menstruate earlier and went through menopause later were less likely to develop dementia. Menopause at age 45 or younger seemed to increase the risk by 28 percent.Another study of 133 elderly women in the U.K. found that the more months of pregnancy they experienced during their lives, the lower their risk of developing Alzheimer’s.The findings all suggest that female sex hormones — which rise at puberty and during pregnancy, then fall at menopause — are somehow affecting a woman’s risk of developing Alzheimer’s and other forms of dementia. The results also suggest that greater exposure to these hormones, through more pregnancies or more reproductive years, can reduce a woman’s risk.But it’s still not clear whether the mere presence of female sex hormones is a reason that the frequency of Alzheimer’s is greater in women than in men.One possibility is that it’s not female sex hormones on their own, but rapid changes in their levels that are a problem, says Pauline Maki, a professor of psychiatry and psychology at the University of Illinois at Chicago, who presented research at the Alzheimer’s conference.”Women experience these very dramatic hormonal transitions that in the long run can give rise to Alzheimer’s disease,” she says.One way for women to minimize the dramatic hormonal changes that occur at menopause is to use hormone replacement therapy.That approach fell out of favor more than a decade ago when a large study found that women who took estrogen plus progestin after menopause were actually more likely to get some form of dementia. They also appeared to have a higher risk of heart disease and breast cancer.But Maki says more recent studies suggest that hormone therapy — especially estrogen alone — really can be helpful if women get it at the right time.”The effects of hormone therapy depend on the timing of use,” Maki says. “Use later in life is detrimental, whereas use early in the menopausal transition could be beneficial.”An analysis presented at the Alzheimer’s conference supports that idea.It found that in two recent studies, women who started taking estrogen after age 65 were more likely to have trouble with thinking and memory. But women who started taking estrogen between 50 and 54 were not.And estrogen may benefit the mental function of younger women because it reduces the hot flashes associated with menopause, Maki says.”The more hot flashes a woman has, the worse her memory performance,” Maki says, citing her own research. “And when we intervene to address those hot flashes, her memory performance bounces back.”Findings like that are renewing interest in the idea that someday, it may be possible to use hormones around the time of menopause to prevent Alzheimer’s and other forms of dementia later on, Maki says.In the meantime, there’s evidence that hormonal differences between men and women may affect their brains in ways that affect doctors’ ability to accurately diagnose Alzheimer’s, Maki says.At the Alzheimer’s conference, she presented research showing that women tend to have higher verbal memory skills than men, even when they are in the early stages of Alzheimer’s. As a result, women are likely to be diagnosed with the disease later than men.It’s unclear whether male hormones, such as testosterone, affect a man’s risk of Alzheimer’s.Copyright 2018 NPR. Looking for a way to battle against a potential crackdown by the Trump Administration on legalized marijuana, California lawmakers have turned to another area where they don’t always see eye-to-eye with officials in Washington.Taking a page from laws creating sanctuary cities for illegal immigrants, a group of lawmakers have submitted a bill that bans California law enforcement from working with federal authorities if they attempt to take action against marijuana grow facilities or dispensaries that operate legally under state law. Doing so would require a court order signed by a judge.Assembly Bill 1578 lays out the goal plainly.The bill forbids local authorities from working with federal agents, including using “agency money, facilities, property, equipment, or personnel to assist a federal agency to investigate, detain, detect, report, or arrest a person for commercial or noncommercial marijuana or medical cannabis activity that is authorized by law in the State of California.”The local agencies include “but is not limited to” police, sheriffs, university police and any state agencies. Under federal law, marijuana remains an illegal drug, listed as dangerous as heroin.Among those who keep reminding the nation of this fact is Attorney General Jeff Sessions, who has made his opposition to marijuana legalization clear multiple times (famously opining that “good people don’t smoke marijuana”). Here are some of his latest comments during a press conference in March.In April, Sessions charged a task force that he created with reviewing lenient federal enforcement policies in regard to marijuana. This also certainly include the Cole Memo, written by an attorney in the Justice Department during President Barack Obama’s administration. If the job growth and economic boon of marijuana that has happened in Denver is any indication, California has a potential billion-dollar industry on its hands.ReactionAssemblyman Reggie Jones-Sawyer, who represents Los Angeles, is the lead sponsor on the bill. He told L.A. Weekly his main goal is to protect the burgeoning marijuana industry in the Golden State, which he called “one of the greatest businesses” in the state.Adam Spiker, executive director of the Southern California Coalition that represents legal marijuana businesses, said the coalition is focused on making sure “the intent” of the bill becomes law. “Cherry-picking when to respect states’ rights and arbitrarily doing so is inconsistent at best and confusing at most,” he told L.A. It’s true that Trump has devoted entire books to chronicling his business successes, but as democrats and others launch attacks on Trump’s business credentials — and question whether he’s really as business savvy as he says he is — another presidential candidate who built his career and his fortune on his business acumen is garnering increasing attention. Meet Gary Johnson, the libertarian party’s nominee for president.Related: Survey Shows Presidental Candidates Miss the Mark on Small-Business IssuesJohnson spent the first two decades of his career building a construction business from scratch. When he started Big J Enterprises in 1976 in New Mexico, he was the company’s only employee. Johnson grew his business into a multimillion-dollar company with more than 1,000 employees, selling it for $10 million in 1999, a few years after being elected governor of New Mexico.Later in life, Johnson started the nonprofit advocacy organization, Our America Initiative, to promote free-enterprise ideals. He also served for a year-and-a-half as CEO of Cannibis Sativa, a Nevada company that manufactures and sells legal marijuana products. In a June op-ed piece, Fortune magazine proclaimed  that Johnson might be “the closest thing the business community has to a candidate” and “corporate America’s best bet for president.”Johnson credits much of his success to the business skills he’s honed and perfected throughout his life. He says his guiding principles for navigating the business world are the same ones that he now uses to navigate the political world. I sat down with Johnson recently to talk about his advice for succeeding in business. Here are his top four tips:1. Always keep your word.Johnson says when you promise something, you need to deliver on it, and you need to do so on time and under budget. When he was governor of New Mexico, he routinely vetoed bills that he believed the state didn’t need and couldn’t afford — 750 in all — earning him the nickname “Governor Veto,” a badge he wears with pride. “Do what you say you’re going to do for people,” he told me. “Given the fact that I did business for thousands in Albuquerque (as New Mexico’s governor), you would think someone would complain somewhere. No one complained because I made right by everybody.”Related: The Secret to Becoming Exactly Who You Want to Be2. Always tell the truth.Johnson says that he has never shied away from sharing the truth, even when it was politically unpopular. He is one of the most high-profile political voices advocating legalizing marijuana, and he’s been vocal about this long before mainstream Americans began coming around to the idea. His approval rating took a hit when he announced his support in the 1990s for marijuana legalization — a shocking position at the time for a Republican governor at the time — but it didn’t faze him. “If you tell the truth, you don’t have to remember anything,” Johnson told me. “It’s hard to do damage to someone who is telling the truth.”3. Always be on time.Johnson views being on time as central to a strong work ethic. He was born in rural North Dakota to working-class parents, an experience that informed his views on work ethic. “Everybody’s on time,” he told the Fargo Inforum newspaper, describing the values of the people from his hometown. “There’s just an unquestioned work ethic… Just the whole honesty, integrity — that’s what was ingrained in my life.” In my conversation with him, he also emphasized the importance of being on time. “I show up on time to a fault.”4. Always admit your mistakes.Johnson says he’s never been shy about acknowledging his mistakes and doing whatever he can to make right by them. At a presidential town hall forum in August, Johnson declared that the Black Lives Matter movement had helped him realize his “head’s been in the sand” on the issue of racial prejudice. “I think we’ve all had our heads in the sand,” he said during the forum. “And let’s wake up. Having a good product at a fair price, and with great customer service, is no longer enough to succeed — it’s merely the cost of entry. And in today’s hyper-competitive environment, entrepreneurs must be innovative.Increasingly, innovation doesn’t just mean building a better mousetrap. It means operating with a new sense of awareness of factors that go beyond profit and loss. “Conscious innovation” is what Daniel Dworkin, partner at Schaffer Consulting, describes as his take on what Raj Sesodia and John Mackey from Whole Foods described as “conscious capitalism,” a method of doing business that looks at the benefits that can accrue to all stakeholders. (In addition to the traditional stakeholders of business owners and stockholders, this also includes employees, partners and the communities in which you operate.)Related: 4 Ways Entrepreneurs Can Practice ‘Conscious Capitalism’“Innovation is a hard challenge,” said Dworkin. “If you take a step forward, you take three back, and you get the doors closed in your face from every angle. You have to fight to get in front of customers to break through the noise, to get resources from your sponsors, and you have to have a deep sense of passion for what your organization is doing, and for the strength of your idea to really succeed. Innovators that work for conscious companies innately have that deep sense of engagement and resiliency to get up, despite setbacks, and keep going down the road to whatever innovation outcome you’re trying to achieve.”You don’t have to take a financial hit to do good.The idea of paying above market rate for goods (as in the case of fair trade goods), paying attention to underserved populations or moving away from command-and-control leadership is by no means Marxism, and the profit motive remains firmly in its rightful position. In fact, in a study by Professor Raj Sisodia, co-author of “Conscious Capitalism,” conscious companies analyzed from 1996 to 2011 outperformed the S&P 500 index by a factor of 10.5.What has complicated the drive for growth and profits is that consumer awareness of what you do and how you do it is at an all-time high. Operating as a conscious company is no longer the stuff of idealistic college students who haven’t yet learned about the harsh realities of commerce and cut-throat competition. Operating as a conscious company today is a meaningful tactic for revenue growth and customer retention and often the foundation of the next wave of innovation.“Conscious companies aren’t just feel-good, do-good organizations,” said Dworkin. “It is directly tied to business performance.” While Wall Street traditionally measures value in terms of return to stockholders, a conscious company measures value in terms of a broader ecosystem. This ecosystem includes owners and shareholders, but also includes value created for the employees and team members, to the consumer and to the community in which the company operates.Employees want more than just a paycheck.What we expect out of a job has also morphed over the years. In the post-war boom era of the fifties and sixties, the Greatest Generation wanted a steady paycheck and benefits, and they got it. Millennials have a different set of goals, and they have redefined what they think a job should be — and more companies, finding it difficult to find new hires, are re-evaluating how they do business. “Talent, retention and recruitment is huge,” said Dworkin. “Increasingly, millennials and the generations that follow are looking for companies that they can really connect with on more than just a transactional basis.”Related: This Is How to Train Your Managers to Boost Engagement With MillennialsIn the effort to see employees as valued stakeholders, conscious companies are doing more to create a new type of work environment where people feel optimally engaged. The way that leaders lead in a conscious company represents a big departure from traditional command-and-control leadership. “It’s the way they inspire and enable people,” said Dworkin. “Their whole frame of reference is to create the conditions for their team to thrive, versus simply telling people what to do and making sure they get it done. You’ll find that companies are actively developing and promoting leaders who operate in this sort of servant-leadership orientation.”Entrepreur Iskra Tsenkova, founder of the Angel Shave Club, a boutique subscription shave club for women, was inspired by Nobel Peace Prize winner Malala Yousafzai, who dedicates her life to helping girls in underprivileged countries obtain an education. “My education meant a lot to me while I was growing up in Bulgaria,” said Tsenkova. “The Malala Fund was a natural choice for me and my business. I’ve been blessed to experience the power of equal education for women, and I believe that every girl and woman should have the right to access that power.”Tsenkova notes the many advantages of running a conscious company. “Every single member of my team feels like they are a part of something bigger and more meaningful. The reward of knowing that the message your company is sending and the cause that it is standing behind is transforming little girls’ lives across the world is simply priceless.”What does a conscious company look like?It can be anything from the stereotypical dotcoms who spoil their employees with nap rooms, free gourmet cafeterias and private buses; to fair trade companies that support third world farmers; or investment funds that put money into socially-aware companies.In the fashion industry for example, new designers need to be innovators in an environment where everybody is an innovator. Styles change so fast, by the time the latest designs hit the local discount chain stores, they are already out of date. They want to be part of a profitable enterprise, but more importantly, they want to be part of an enterprise that is doing something positive in the world. Drone sighting halts flights at Irelands Dublin Airport

first_img The airport said the Irish Aviation Authority suspended flights for 30 minutes after a confirmed sighting by a pilot at around 1130GMT.”As there were no further drone sighting within the 30-minute suspension period, Dublin Airport resumed flight operations shortly after noon,” the airport said in a statement.It said three incoming flights were diverted to other airports during the closure, and some knock-on delays to departures were expected for the rest of the day.The airport said “the safety and security of customers is our key priority at all times.”The popularity of drones makes them a growing threat to aviation.More than 100,000 travelers were stranded or delayed before Christmas after drone sightings near London’s Gatwick, Britain’s second-busiest airport. No one has been arrested over the incident, which saw the airport shut for parts of three days. We don’t know whether they were PLA personnel since they were in civil clothes. Those protesting were on the Chinese side and made no attempt to cross over,” said a source. He said Naidu was claiming a great achievement by giving just 5 per cent money.There were repeated interruptions when the chief minister was speaking. Irked over this, he remarked “We are 151. If we decide you will not be seen around.”When TDP leader Atchan Naidu tried to intervene, Jagan Reddy asked him to sit and taunted him. “The man is of this size, but the brain is not of that level. You don’t know what to speak and what not to speak,” said the chief minister.Speaker T. Seetharam also reprimanded the TDP members for repeatedly creating disruptions.YSR Congress Party (YSRCP) has 151 members in 175-member Assembly.
